No we have already investigated the list you have above.
Always above 25% MC and usually sufficiently (stationary) units in the province.
Most of these Def works orders occur in rear echelon areas so there is not really many cases of moving units or enemy present.
If you look at the last attached log and read the messages you can see the difference.
So messages 58 to 65 record failed def works attempts. These 7 in my view are a bug (i.e. they meet all criteria and there is no reason for them not to be enacted.) They all read " An attempt to perform the decision Build Defensive Works in regions "x" was unsuccessful".
Now where there are genuine reasons for failures (units moved out etc) it appears there is a different message (as in message line 56 )reading, "The decision Build Defensive Works is not valid: Not enough elements of the correct family".
So you can see this attachment from last turn shows 1 genuine failure and 7 unsuccessful attempts that should have been valid.
This is happening with almost every player in our group and it is occurring "every turn"!
Players home regions are now reaching high levels of rebellion as players have limited toold to quell rebellions without a functioning Def Works card.
