When we redesigned to "new Boston" region with patch 1.04, the surrounding regions [The Heights, Peabody] now represent the 'suburbs' that previously were incorporated in Boston 'size'.
The intent is to create the 'historical siege stress' on the British [or whomever
] occupying Boston by reducing available supply. Previous to the redesign, the Americans couldn't sustain a "true" siege very long, as they couldn't be supplied through winter. Now, the Continental Army can 'fortify the Heights' and build a depot. The presence of a fort on the Heights will also bring enemy shipping under fire as it transits in and out of the new 'Boston Harbor' region.
