All locations generate supply each turn not under siege. When a siege starts, there exists a 'pseudo stockpile' of 1 turns supply inm place. This pile then is consumed without replenishment as long as the siege reamains uninterrupted.
Fort does not confer immunity.
If there were a harbor, not fully blockaded, there would be reduced supply generated each turn...
Did besieger sustain uninterrupted siege? Did the besieged move troops outside for a turn to break the siege [sally]? Is the besieger always avoiding Passive stance?
I did quick test of a siege, and it seems OK. Supplies reduce slowly inside, but do get to 0 [when no port is there]. Rate of reduction depends on how many troops are inside [consumption per turn]. More likely is a surrender before then if only low quality troops are inside.
In the end, it's hard for us to say what you've got without a saved game to look at.....
