How to form division, stupid noobie q

Ok, I have now run the demo campaign a few times (twice as USA, once as Confederation), and I think I have the hang of things (and will buy this as soon as I get to work where I on Friday happened to forget my suitcase (and in that suit-case is my wallet, my mobile phone plus some paper-work I intended to work with during the weekend, but happily I have now had time for other things as nobody have disturbed with silly calls and I haven't had the paperwork at hand
); notice: The fact that I already had a couple six-packs in the freezer for the weekend (and thus didn't have to drive home via the shop to buy some) has nothing to do with the fact that I didn't miss my wallet until Saturday).
However, there is still one thing that confuses me over and over again.
What exactly are the conditions I need to fulfill to make a one (or two or three for that matter) star general a division commander? I have had several times two commanders in same stack, and despite shuffling troops around, sometimes only one of them is willing to be a division commander
Note, I haven't been even close to the 30/60 limit.
Which of the following need to be fulfilled for a commander to be available for division commander status:
1) Need to be active
2) Need to be un-locked
3) Need to have a brigade/regiment with him
4) Need to have a supply-wagon with him
5) The moon must be in correct azimuth to Mars
6) Something else that I have completly missed
????
As it is, I seem to be able to *never* make some commanders divisional, for example my hero Longstreet refuses to become a divisional commander even when others in the same stack and same troops. I shuffle things around, give and take divisions from commanders, and I don't seem to find the logic.
It would appear it is much easier to make army commanders and/or corps commanders than make one-star commanders divisional-commanders, so there must be something crucial I am missing
