At some points in the game I will go through an entire nations force by the number using the a (next) key. That way you are sure to get them all in one pass. At other times during the game, such as the beginning of the GC I will deal with the armies I have big ideas for and then take a look at the generals/army tabs on the right side of the screen. Those with orders have the envelope-one with a lock don't matter. By clicking on the general the map is centered on that force. You can then update that army's orders. If you still have many armies to issue orders to after you finish your initial actions, it is best just to use the A key and roll through them all.
An advantage of the A key is that when you go to the next army, it is already "selected". My preferred method for finishing up on issuing orders (sometimes at the very start of a quiet turn) is to keep one hand on the keyboard and the other on the mouse. I click A, then "activate" with the mouse' then "execute" (arrow) with the mouse. This takes about 2 or 3 seconds per army.
When you are finished with one nation under your control just click on another one of your nations on the mini map, then click on the actual map and the appropriate generals will appear on the right side of the screen. Chose the first army and activate it, then switch to the a key and mouse routine. You will be surprised in how quickly you can get through a military phase like that.

I cannot say that I have had this happen before. I have ended turns with friendly armies in the same province at the end of a turn, but not opposing armies-at the end of a turn. I have had enemy forces in the same province after a battle, but before a breakthrough battle.
So, I can't help you with that one, but I will certainly keep an eye out for it in my games.
