If the fort system reflects that present in AACW, I believe that once a unit is inside a fort, it cannot entrench at a higher level than the fort. Not sure what the algorithm is, but there certainly is an advantage to bigger ports, etc. The upgrade to Naval Base especially is very pricy. Forts and Entrenchments are two different things. Did some checking, here's a few more details:

Happened on early July 1851 turn (date was wrong in OP).

Cause was Uncle Tom's Cabin event.

Result was: Loyalty in Washington DC went from American's 100% to American's 20%/Rebels 80%.

Military control remained at American's 100%
